$200 Switcher Gift Card Rebate Code not working

I recently purchased 4 phones at Best Buy the qualify for the $200 Switcher Rebate.

I just went through the online rebate submission and the code I was provided is not working. I am gettting this error message:

Promo Code with the Order/Purchase date can’t be found. Please check your information and try again.

Please confirm how I can receive a code that will allow me to submit my rebates.
