Can't Redeem 350$ Samsung Fios Home Offer

I want to redeem the samsung offer (from getting Fios). Originally it was a chromebook, but now it says I can get a $350 samsung credit for any device. However, when I attempt to redeem it I get sent to <pictured> page on samsung with seemingly no way to redeem or access the credit. When I check something out, there's no credit applied to my cart. Not sure what to do, thanks.
