Please provide terms and condition for below offer, it say it applies on cart, but I don't see any offer related to switcher rebate on my checkout page.

Here's a thread from someone who managed to get their Switcher Rebate earlier this year. Plenty of details. Try all of the three codes they gave to enter at the website; hopefully one of those works for you:

I don't know about the free phone thing. It's my understanding that discounts and promos can't be stacked. So if none of the switcher rebate codes are working, it's possible you only got a phone deal where Verizon gives you a bill credit for 36 months until the credits add up to the cost of the phone.
