$200 switcher rebate

I switched to Verizon on October 27, 2024. I ported from T-mobile and purchased phones online. But I did not received my $200 switcher rebate.

I called customer service they sent me a link where I can claim but I lost the PROMO CODE OR PROMOTION CODE where we claim on rebate center website. Please help me with that code so that I can submit my request for $200 credit.

I got the code 200SWITCHNOV24 which doesn’t work and obvious reason I switched on October not November.
