Got notification about submitting rebates, need help finding the promo code

On 1/11/25 I purchased 4 phones with Verizon switching over from T-Mobile. I purchased these on the phone for in store pickup. I gave the rep all the info from my T-Mobile bill. I don’t know if this rebate is for the pay off my phones deal or the $200 gift card. Can anyone from Verizon help?
