LTE network extender is inconsistent

I've got poor cell service at my home and wifi calling wasn't great with my Unify mesh network so I got a 4G LTE Network Extender.

I have frequently have poor audio even when making calls in my office which is 5 feet from my extender. It's not all the timne on a call, but there's always a part of a call where I sound bad to the person on the other side or their voice sounds bad to me.

I checked and it's always connected to between 5 and 8 of the 12 satellites.
Yes, I have wifi calling off.
I called #48 and I am connected to the extender.
I use a Galaxy Fold 7, fully updated.
500 Mbps up and down via AT&T.
Problem happens regardless of whether I'm home alone or the kids are streaming Netflix.

Any ideas?

Check the "MTU" (Maximum Transport Unit) size in the extender's local admin GUI settings. This can affect the call and data quality.

Press the button on the left side panel near the screen until you see an IP Address displayed.
From your computer that is connected to the same home network as the extender, type the IP address into the browser's URL bar and enter.
Now you should see a login page. The pw is LTEFemto0000 (with the zeros representing the last four of the MAC Address found on the back label or the screen).
Once logged in, from the left side menu select 'Settings' and then from the drop-down menu select 'Network'
Look for MTU Size. The range is between 1380-1500. Lower the number could = more bandwidth initialized/lower speeds and higher the number could = more latency/bad call quality. Try changing this value size.

Also, another thing to check to see if your modem/router allows you to change the MTU and QoS (Quality of Service) settings.

I have this same problem. I had the older style network extender for almost 5 years and it worked flawlessly. Verizon said that it was end of life and upgraded me to the new one and I have had absolute garbage call quality ever since. Tech support is useless and just keeps sending me new ones. I am on my 3rd one and all 3 have the same problem as described by the OP.
