Galaxy S4 Lollipop Update: Unable to place calls. (Data, SMS, MMS, Wifi, GPS all work fine)

Recently upgraded to 5.01 and everything works except for the ability to place a phone call. Never connects to anything and eventually just says "CALL ENDED"

SMS/MMS both work. Data/GPS/Wifi all work fine.

Ensure that network mode is set to LTE/CDMA and not Global. You also might try removing the SIM card for a few minutes (be careful not to touch the gold contacts).

I've tried all 3 modes and I get the same result - but yes i am currently on LTE/CDMA. I've also removed the SIM card overnight - no change.

I want you to make a call out mjbosscher! How many bars of signal do you have? What zip code are in located in? When you make a phone call does it go straight to call ended? Are you able to receive calls?

I'll go one further. See attached screenshot.

Right now I'm 92612 but i've had this problem all over.
When I try to place a call it says DIALING for probably 10-15 seconds and never connects to anything (no ringing) and then eventually it just says CALL ENDED.
When someone calls me it goes straight to voicemail.

That screenshot is extremely helpful. Thanks, mjbosscher. While some of the steps in the link stef7 are helpful, let's try having you place your phone into Safe Mode http://www.verizonwireless.com/support/knowledge-base-77504/ and try to place and receive a few calls. While in Safe Mode, do calls connect quicker? Are you incoming calls received?

Safe Mode = Same Situation.

Unable to place or receive ANY calls. My solution has been to flip my sim card into my old junk HTC Incredible when I need to make a call. Looks like June4th the LG G4 comes to verizon so I only gotta make it a couple of weeks - then I can get a new phone and shop around for the best carrier plans.
