Why won't my camera focus?

I have a Galaxy S6 and I have been having a problem with the camera not focusing correctly. When I start up my camera app the screen is blurry and out of focus. Tapping the screen to focus has no effect. If you shake or tap on the phone hard, the camera will finally focus and work fine until you close the camera and open it up later.resetting the phone does not fix the issue.
