Several times recently my Note 4's ringer has turned Ringtone, Notifications and Sound mode off by itself. Any ideas about why this might be happening? I have not turned these settings down myself.

That would concern me too! I count on my phone to let me know when people are trying to reach me. How often does this happen? When did you first notice it?
