What make/model 'signal booster' do you have? Generally, you could call Customer Support at 800-922-0204 where you may have to request transfer to Tech Support for help.

To add to that, 1) if the 'Signal Booster' registration form doesn't work, you’ll need to reach out to Verizon support for them to manually register it with the booster's information to your account. 2) For the 'Verizon LTE Network Extender' (ASK-SFE116) it's typically associated automatically and activated to your account when you order it from Verizon directly and shown in My Verizon Website's 'Manage all devices' page. Therefore, the device would display 'In Service' after setting it up. If that's not case, you'll need to have Verizon support manually activate it by the MAC ID found on the back label or display screen.
