Looking to get ahead of this

I switched over from TMobile on 12/22, the Verizon Agent I talked to today couldn't find anything about what I needed to do to prompt this promotion. 

Can anyone let me know what I need to do on my end to make sure that this goes smoothly ?

After reading through the pages it appears that it will be an email most likely in my junk box, that I'll be looking for. 

Again , Until the 90 days plus potential 14 weeks...it there anything I need to do. 

I saw an earlier reference to visit this site: https://www.yourdigitalrebatecenter.com/#!/ 

But I don't have a promo code to move past the front page.
