Conditional Call Forwarding No Longer Working

 We had conditional call forwarding set up and working but yesterday I received a phone call that was forwarded before it went to voice mail.  There have been other calls since that are forwarding when the call is not missed. In other words, the "Conditional" part of it isn't working. It's just forwarding everything. 
 
I have a Samsung Galaxy SS22 Ultra
