Fios DVR app

Recently while troubleshooting my phone w/ a verizon rep. I had to reset my phone. My apps came back but my Fios DVR app will not let me log back in. I've no idea what my username/ password are. I did it once, and forgot.

 

How can I reset this app and get it working again?
