File a formal compliant against Lake Zurich store

Hello,

I would like to file a formal complaint against the Lake Zurich, IL store. Is there a mailing address, phone number, email address, or online form?

I have been a customer for over 20 years: The experience I had at the Lake Zurich store over the weekend was awful, disrespectful, and demeaning. I need someone at Verizon to address the situation.
