I've heard of this problem occurring if you bought the phone from a 3rd party, and not directly through Verizon. Another thing which is problematic is if you're not the original owner, because it is possible for a phone to become permanently locked to a carrier for reasons I'm not allowed to discuss in detail. If you're the only owner of the phone, you'll need plenty of evidence showing you legitimately purchased the phone for a SIM Lock Appeal to be successful.
I'm not a Verizon employee, just another customer trying to help.
