My 4620 has attempted and failed three firmware updates (here is the last log entry):
Can someone please tell me what might be wrong here?

Me too! Very frustrating.
Tried a hard reset before updating - no change.
Hoping for a solution soon - I hate having updates hanging out there... but my device is working great!

Yeah, I tried everything here also. However, I just read in another 4620 forum that we aren't alone on this issue. And that we might have a fix as soon as the end of this month.
10-11-2012; Just talked to Tier 2. Awaiting a fix which will be 3.20.10, no eta. thats nice. All solutions found in Broadband web site fail also.
