I had my phone just a little over one month. Video / Pics etc on the SD card. All worked just fine.
I Even purchased the docking station and have a windows 7 laptop I plug into for charging. Not sure if any of this is causing the issue, but now my phone states Blank SD Card.
I too have tried to use the phone to Format the card, taken the card out and put back in, same thing. Blank SD Card.
I have taken the card out and put it into a PC. PC sees the card, but then asks to format it. I then tried that, but nothing.
I did notice that everytime I was plugging the phone it the SD card was mounting / unmounting when taken off plug.
Maybe it's stuck somewhere in-between? (Mount / Unmount)?
Ok, so the SD card is bad. What next....
1. Called Verizon to discuss issue, they have a Blanket Statement that says they do not warranty the SD Card. But call Sandisk and they will warranty it for 5 years since Verizon used SANDISK sd cards.
2. Spoke to two different people at SANDISK. Since the Memmory card does not have the SANDISK logo they will not
warranty the SD Card.
3. Called back Verizon and explained the issue of the card not having the SANDISK logo? They had no solution except to tell me the card is manufactured by Sandisk and they should honor the warranty. NO HELP!
So where am I, yes in the middle no support from either side.
What next, bug the Verizon store until they replace my SD Card I guess.
Any suggestions?

Sounds like you may have a corrupted error in SD Card, when you format card did you to a complete format or just the quick format? If you did a quick format place it back in pc and run a full format and then place back in phone and then when you plug in device follow my instructions to fix connection issues located here... http://community.vzw.com/t5/DROID-X-by-Motorola/USB-Connection-Issue/m-p/326834#M14519

Using your phone to format it will only do a quick format. You should follow Wildman's advice about formating it with your computer.
As for the card being warrantied, it's a slippery slope. Your best bet is to take the phone to your local Corp store and tell them the problems you are having. It's hit or miss if they replace it for you but it's worth the try.

I had this exact same problem about a week after I got my phone. Motorola ships the SD card with the Droid X, so it is not a Verizon SD card from Sandisk which is why it does not have the Sandisk logo on it. Motorola buys these SD cards in bulk and there is no way to tell what vendor they come from unless Motorola would tell you. I took mine to a Verizon store (has to be a real corporate store) and they swapped it out with no charge for a brand new Sandisk SD card. I've had absolutely no problems after that. It's a problem, Verizon knows it, as it has happened to many, many people. One card returned to the store was black on one side and lime green on the other! Get Verizon to swap your card and you should be fine. Of course, you'll have to reinstall your apps and such, but you'll be good to go after that.

I just noticed something.....when my phone is pluged in to my computer....while using astro file manager....it says that my sd card is blank. Then i unplug the phone and it says prepairing SD card and then i can see all my files again. So it seems that if the phone is plugged in, some how the sd card is disenaged or something.

When you mount the card in device as a Mass Storage the card will dismount from the phone to allow you to R/W files to and from files and when cable is unplugged it will allow the card to be read by the phone again, the card can not be accessed by both the PC and Phone at the same sound so what you are describing sounds like its functioning correctly. If you format card the card would be empty but once you unplug phone the device will place folders into the card for system files.
