I just got a replacement phone after one month the. I set up the replacement phone. How do I erase all the data off of my old phone so I can send it back?

Follow the instructions located at http://support.verizonwireless.com/clc/devices/knowledge_base.html?id=46649 that will allow you to hard reset the device and restore it to the stock out of box state...
Make sure to disable the Auto restore option to avoid your data getting restored automatically..
Preferred method
- From a Home screen, select the App menu (located in the lower right).
- Select Settings.
If the application icon is missing, select App menu filter (located at the top) then select All apps.
- Select Privacy.
- Select Factory Data Reset.
- Select Reset phone.
If presented, enter the current passcode or draw the unlock pattern.
- Select Erase Everything.
Allow up to 3 minutes for the factory data reset and reboot processes to complete.
- Refer to Initial Activation and Setup.
