While trying to get rid of "visual voice mail" on my Motorola Droid Ultra I lost the voicemail icon on my main screen. How do I reset it to JUST have basic voice mail?

As Ann154 said, the icon is for the VVM app; you can set a speed dial for *86 to access the basic voicemail; set up a contact called Voice Mail, and put the speedial for that contact on your homescreen.
Icon for the voicemail app is meant to access the Visual Voicemail system and not the basic non-visual voicemail.
