Sending and Receiving Calls I hear a LOUD Dialing Noise

When I receive a phone call, I slide the phone activation answer screen (to answer the Call) and I hear a very loud (5 or so key presses) noise before the call connects.

I get this SAME noise when I make a call.  I dial the number (or via contacts), I place the phone to my ear and just before it rings I get this LOUD dialing tone sequence.  This is very annoying.  I now "wait for it!!" before I place phone to my ear!!!!  How can I correct this?

I've done a battery pull twice and the phone still makes the noise.

I dont know how to do a soft reset - is this the battery pull?

How do I do a hard reset?  Will I loose apps, etc.?

Doing a Hard Reset fixed the problem..  

Hard Reset:

Menu/Settings/Privacy/Factory Data Reset...
