need a puk code to unlock

My son has something on his screen that says enter puk code to unlock, it says 1 attempt left.  How do i get a puk code?

Check your My Verizon page for that line.  If it isn't there, call CS.

Im so  upset ....................... My phone is also saying i need a puk and at the top of my phone it says locked sim ..... i cant get a phone call or make one not even to VERIZONNNNNNNNNNN To fix the issue  HELP PLEASE ... Thank you

Can you log in to myVerizon online? If so, you're in luck. From another user's question thread:

"If you sign into My Verizon from a computer, you can find the code there.  Once you have sign in roll your mouse over the My Verizon header and select View All under My Device.  It will show you a picture of the TB and at the bottom of the picture you should see:

Unlock SIM (PUK) now

Click on the "Unlock SIM (PUK) now" link and follow the instructions"
