where do i find my puk code number? i've had this phone for 2 years and i have never set up a puk code, now i cant access my phone or make outgoing calls and i dont know if i can accept any incoming calls, and my phone was just upgraded yesterday, the look is totally different what do i do

Call VZW from a different phone, they can give it to you.

I think it's a problem related to the ICS update.

Go to My Verizon and enter your Account Info and Look for Puk code should bring it up Or as one Suggested call Customer Service 1-800-922-0204 Option 3

The PUK code is required when the sim card password has been inputted to many times... You can go to https://login.verizonwireless.com/amserver/UI/Login?realm=vzw&goto=https%3A%2F%2Fnbillpay.verizonwireless.com%3A443%2Fvzw%2Faccountholder%2Fservices%2FdisplayPinPukInfo.action and click the option to reset PUK and put in the number that the phone shows you for your card and it will give you a default unlock PUK code for your device.
