Incoming calls to Fios landline randomly misrouted

When I call my mom's FiOS landline, often the call is misrouted somehow. This happens to other callers too.
What happens is: her phone rings, and she picks up. But she is not connected to the caller--instead, it seems that she's randomly connected to some other endpoint. Sometimes there's a real human on the other end, sometimes an answering machine, sometimes it just rings or there's silence.
On my end, as the caller, I experience the same thing: I get a random answering machine, or someone picks up at another number.

As in their case, this is a number that they've had forever, and was previously associated with a copper landline. Also, I've confirmed in several cases that the caller's provider is t-mobile or a provider that uses the t-mobile network (but I'm not sure that's true of all callers).
We've made multiple calls to Verizon support, and a couple techs have come out to visit (though it doesn't seem likely to be a problem with anything in the house). I also have a case open with my own provider (Google Fi). There's been no progress on any of those fronts.

As you noted, this happens to me too! So far the Verizon tech has come out to the house and found nothing wrong with the wiring.
Interestingly, he "reset the switch at the CO" and temporarily everything was great! People who previously hadn't been able to get through to the line were suddenly able to do so! However, that lasted one day, and now we're back to the original behavior.
It sure seems like there's something wrong at Verizon.
