DVR goes Black

While playing my DVR'd shows, WHEN FFWDing, the screen will freeze and turn black
After about 60 seconds, the DVR cuts off and goes back to Live TV
When I go back to DVR I have to start my shows over

When the television screen freezes and goes black, is the STB rebooting?
If it hasn't been tried, unplug the STB, wait a minute or two, plug it back in and see if that makes any difference. There is always the chance that the DVR is malfunctioning and in need of replacement.

All the Fios DVRs used hard drives for DVR storage, your drive may be going bad. If doing the power cycle doesn't help try this, and if that still doesn't help, I'd watch all the shows you have, and call support for a replacement keeping in mind you'll probably lose all your recorded shows.
