Please help! 

I’ve uninstalled. 
I’ve cleared the cache.

I’ve reset my password. 

no matter what, it says account disabled when I try to login to the app with the following error code. 

please help!!

Try calling billing and have them reauthorize your account, then reinstall the Fios app on your Firestick.
