Twice I asked for the box and label to return a LTE equipment

Ok . Verizon put it together…I upgraded to the FIOS. Service on my new address. I cancelled the LTE on my old address in Feb 7…twice I have been waiting 2 hours on the phone and you guys are supposed to send me the box a label. 
Now not only didn’t do it BUT also sending this warning about getting a 200 dollar fine for your incompetence??? 
this is besides annoying very ridiculous, what I am going to use a LTE box that didn’t even work well ? I  need to send this back asap, don’t want another people’s trash on my apartment.

should I go to a store ? What can I do ? (This is an email after 2 calls in 15 days, for the records)
