Hello, I transferred from T-mobile to Verizon for 4 lines. Ported the number and got the iphone trade-in offer. But I do not have details for $200 switcher rebate for each line. How do I submit it. Also, I have heard that there was a free apple TV and 6 month subscription,, what are the details for that offer
