Was not given $200 Rebate Promo Code for switching from T-Mobile

I just switched to Verizon from T-Mobile on 11-29-25 at a Verizon authorized store.  I was told and the receipt says i have a $200 rebate. I went to the link provided on the receipt but is asking for a Promo code to redeem the rebate. I was not given promo code on the paper receipt or through email. 

How do i get this code to redeem this rebate?

Awesome1 congrats on the new phone. Go to the My Verizon app:

  1. Access rebate forms through My Notifications > in App Alerts or Shop > Orders Rebates.
  2. Review the information.
  3. Click Submit.

 

Result: The customer receives the  Verizon Gift Card via email within 8 weeks of submission.

I see nothing in My Notifications.  Nothing in shop or orders related to rebate either.  Please advise.
