HELP, phone lost my ringtones, pictures, notifications, and music.

I have a Droid RAZR MAXX with ICS 4.0.4 installed.
As of yesterday I have lost all the custom files I had put on the phone.
They are still there as I can use the file browser to see them, but the phone lost them.
I have tried other online suggestions of deleting the Media Store Cache and having it rebuild the database.
I have also done a factory reset and this still has not fixed the problem.
I have also tried the app ScanMedia to no avail.
I have the media in the root dir under Notifications, Pictures, and Ringtones
Anyone have any idea on how to fix this?

Try placing the files in the directory SD Card / media / audio / ringtones or notification (Depending on the file you want to use media for) and then reboot device.
Photos doesn't need to be in a specific location so maybe you should try QuickPic from https://play.google.com/store/apps/details?id=com.alensw.PicFolder&feature=nav_result#?t=W251bGwsMSw...
Also you can try Clearing the system cache...
Make sure you back up your data before doing this!
- Power the phone off
- Power On while holding down the Home key
- When exclamation point icon appears press BOTH volume keys
- Use volume keys to navigate to "wipe cache partition" and tap the power button to select
- Wait for it to finish and select reboot system

A big Whoops Wildman i didn't think you had to back up the data doing a (System Cache Clear.! i thought that was for a F.D.R only) I never did.. maybe i should have though. Not backing up the Data is that what can cause the Apps to act up then. or Something in the System.?
