Haven't gotten my Fios Welcome Gift Card

I've been trying to claim my $50 Fios welcome gift card, but I haven't had any success so far.
On March 24, the gift card tile on my Verizon account offer showed "Redeemed", and I received an email the same day saying the gift card would arrive with 7-10 business days. However, it's now well past that time frame and I still haven't received anything.
I contacted Verizon support, and they referred me to BlackHawk at 833-889-8251 / 888-397-0765 to follow up. I've tried, but still haven't been able to resolve the issue or get any clear answers.
Has anyone else run into a similar problem? Any advice on how to actually get the gift card delivered would be appreciated.
