Process to move Verizon's line from old utility pole to new utility pole?

There was a new utility pole put in front of our house to replace the old pole. All of the other lines were moved to the new pole except for Verizon's. What's the process to have the lines move to the new pole? We would really like to get the old pole removed, as 2 utility poles and a dug up sidewalk isn't the safest for our toddler.

I have the same problem but its worse. The old pole is no longer secure and can be moved in the hole with just a hand push. I want to document this case and my notification so I can hold Verizon responsible if there is damage to my property when the pole finally falls over
