Returned Phone-No credit and being charged full price on the new phone

I returned my phone and I have verification it was received at the dock in Fort Worth.  I still have not received my credit on my account and I have to call in each month to get a credit for my returned phone.  Who can I contact at Verizon to resolve this issue?  The confirmation of receipt is on my account.  This is the proof:

Hello, CaliGal2, thanks for reaching out in reference to your returned phone. We certainly don't want you to have to continue to reach out about the same issue. Why was the phone returned? Can you tell us more about what's going on as far as the charges and credits? Did you receive an extra charge on your account due to the issue with the return and that's what you're looking to get credited for?

I returned the phone because I upgraded in November and sent back my old phone.  I was told that the warehouse had my phone but they never finished the process to indicate they have it.  I was to get a $300+ credit on my old phone and a credit towards my new phone each month.  I am now getting charged each month for the phone value without getting a credit towards the phone I return.
