After five months of use, my phone became very hot today and lost its charge very quickly. What is wrong and what can I do?

What was the temperature of the phone when it was "hot"?

Above room temperature both front and back, very warm to the touch. Enough that the inside of its case also felt warm. It worked fine for calls and tests but quickly depleted its charge. I'm now charging it at home and it is slightly warm.

My Tbolt is currently at 93° F.
I don't worry about it until it reaches above 106° F.

Hello mackee. Lets get to the bottom of this! Does the device warm up when its in use or on the charger? Are you currently using a task killer? Please provide additional details so that I can better assist.

I know this might sound odd, but I have heard that if you have issues with HOT phone to WIPE the phone to factory and this should fix it. Did this start after installing a specific app/apps I have seen a bad app do this and if you can remember what it was try uninstalling it as well...

To eliminate a possible app as an issue, start the phone in safe mode and see if it heats up. In safe mode 3rd party apps are not available.
Mine gets hot occasionally when I have a poor signal and that will eat up a battery
