That does look a little strange! Let's make sure that we can get your device updated without any errors. Have you check on the phone itself to see if it is up to date with the latest software? You can do that by going into settings, More, Software Update, Check for new software update. Please keep us posted.

Both of my SG4 received the latest update. That's when the problem stated.
Baseband version = I545VRUFNG6
Build number = I545VRUFNG6

Thanks for double checking that updated information, gpeterden. Are you experiencing difficulties with the new update aside from the Software Upgrade Assistant? Generally, the Upgrade Assistant should only be needed to update for an older version.
