Hi there johnny1972!

We absolutely want you to get back in business with your device and services. It seems as though the sim pin may have been entered too many times incorrectly and now is requesting the PUK (Personal Unblocking Key) code. As my friend Tikibar mentioned, if you have a 4G LTE device you can retrieve this code by logging in to your My Verizon account and following these steps:

- Click on the image of your device
- Choose Manage My Device
- On the right hand side Choose My PIN and Unblocking Key(PUK)

This will display your PUK code and your default PIN. You will enter your PUK code then enter in your default PIN, most often twice to confirm.

If you have a 3G global device we will need to view the PUK code via our internal system. Please let me know if this is what you need or you can contact Customer Service at 800-922-0204 or via Chat: http://bit.ly/RMU8b0

Please keep me posted!

Thanks!
