My sim got got randomly locked on my Samsung Galaxy s4 and it says on the lock screen to enter my 8 digit sim pin (PUK) can someone please help

Locate the 4G SIM PIN/PUK - My Verizon Website

This may help if you're unable to send/receive calls or connect to the Internet.
Note Only the account owner or an account manager with full online access can perform these steps. Refer to Account Roles for additional info.
Navigate: My Verizon > My Device > View All.
Click My PIN and Unblocking Key (PUK)* (located on the right in the About My Device section).

If I’m not mistaken, you should be able to take the phone to any Verizon Store and have the SIM Card replaced, for free.
