Signing up for Netflix and Max Promotion

Hello - I recently signed up for the 1 Gig internet service that comes with Netflix and Max for 1 year. How do I sign up for these services? Is there a separate login to take advantage of this offer? I do not currently have an account with either service. If I create an account on the providers website how do I link it to the free offer?
