$200 Gift Card Online Popup Offer

I upgraded to the iPhone 15 Pro and the Unlimited Ultimate plan. When I was checking out, there was a popup offer for a $200 gift card. I have a screenshot.
It said: "You'll be able to redeem a $200 Verizon Gift Card in My Verizon after placing your order. Submit within 60 days and a Verizon e-Gift Card will be sent via email within 8 weeks and after claim validation. This offer is online only."
I clicked "accept offer."
This was three weeks ago. It was the reason I ultimately completed my order. I've received nothing via any form of communication or in any app.
Now I feel defrauded. I've talked to very friendly reps in person, over the phone, and over chat. They are all clueless as to this offer. It's an "online offer" which apparently means they know nothing about it.
I've checked the app and my offers section, still with nothing. Nothing in my email or spam folder, nothing in the mail.
There are countless threads about this same issue here and on r/verizon. How do I obtain this gift card? It was clearly designed to get people to complete their order, which it did, yet I have received nothing and neither have many people.

I submitted the Notice of Dispute form, which I found elsewhere, and a very helpful person resolved my issue over the phone.

I appreicate you clarifying those details. I did a little digging and found there's a $200 online offer, to gain a Verizon giftcard, for addtion of new lines with a smartphone purchase. This can be when someone is forming a new account, or adding a line to an existing one. Regretably, I see that upgrades at this time aren't eligble. Which would explain why there's not be an additonal email about the offer sent, nor anything in My Verizon.
I'm sorry about that, but glad I could clarify and confirm the offer details.

Respectfully, the popup appeared as an offer, and I accepted it. It was the reason I went through with the upgrade.
Verizon cannot retroactively say an upgrade isn't eligible after extending an offer. It doesn't matter if the offer was a mistake. It happened, I accepted it, and I have a screenshot. I spent money because of this offer.
I understand that you do not make Verizon policy but this response is unacceptable. Please tell me how I am going to obtain this gift card.

This form in the Verizon Wireless Customer Agreement leads to an error page: https://www.verizon.com/about/nod/notice-of-dispute-wireless-form.
Can someone please provide me with access to this form if you are not going to provide the gift card, which again, was promised as consideration for my purchase?
