Trade-in promo credit trade-in method

I recently purchased 5 lines with new iPhone with Trade-in promo ($28.xx/month 36 months) via chat

My order has split address: 2 devices send to CA, and 3 devices send to TX. I was expecting the agent would give me 2 UPS labels for the trade-in devices similar to the shipping setup.

However, when I received email from Verizon regarding the trade-in, it only provided me 1 shipping label to ship all devices for trade-in.

Is there a way get 2 UPS labels instead? Or can I just go to the nearby Verizon stores to do the trade-in?

this quote "My return method was to ship, but I didn’t use the Verizon-provided label or shipping kit." from here Device Trade-in Program Terms & Conditions | Verizon Support seems suggesting that I must send the trade-in device using the shipping label in order to get the promo credit. will it be voided if I take the trade-in devices to store?

From reading a lot of posts about this topic in the forums:

1) You need to get a shipping label for EACH device you're sending in.

2) If you send back all 5 items in one box with one label, you'll likely get a trade-in credit for only ONE device and the others will be lost in the shuffle.

3) Stores may or may not take trade-ins. I've read conflicting information, and sometimes things still get "lost". If you want to ask someone at a store, I'd use the Stores link at the top of the forums page and locate the closest corporate store to you, then call the store directly.

4) Check your My Verizon account and see if you can print shipping labels yourself from there. If there are multiple accounts being represented with these 5 devices, ask the other account holders to check their My Verizon accounts too. For some reason, getting shipping labels from someone at Verizon is some kind of puzzle they can't seem to figure out?
