I ordered FIOS but I never finished filling out the rebate web form... Is there a website or some process to fill it out online?
I cant find my paperwork

As I recall the $150 Rebate was automatic as part of signing up.
I stared receiving emails from them as soon as I signed up all the way thru when I qualified after 31 days of service.
Not certian if this changed on 06/21 with the new Netbook Promotion. By the date of your post it looks like you signed up on the 20th which was the last day for the $150 Rebate.
