vzrewards not working

The link in the homepage to register for the reward/rebate card is NOT working.

How do I register for the card? I do not want it to expire.

Also, I cannot find any way to contact/email Verizon support regarding this. I don't have the time or patience for "chat" - I would prefer an email or email-like contact method.
