Damaged Verizon utility box

We have a damaged Verizon utility box near 13xxx NE 139th PL, Kirkland, WA 98034.

The utility box cover is broken and there is a hole left in the street which is now covered by a stone, but it still remains as a hazard the children playing nearby and people walking.

Could you please send someone to fix this
