Verizon pulled credit when I discontinued service at address

I cancelled my service 2months early and paid the termination fee.

My ex then signed up at the same address and they pulled my credit despite her using her name and dob (she didn't enter Ssn)

this is an unauthorized pull on my report and needs removing
