Where is my $150 gift card?

I was promised a $150 gift card for switching to verizon fios and haven't received it yet.

I've called numerous times to no avail. The install date was Feb 13

this is the 3rd different number i've seen that's supposed to be for this issue. hmmm... I'll report back after i call it (not hopeful).
