I would try to print it out, fill it out and bring it to a Verizon Fios corporate store location and not a reseller. Have them confirm the move and non service at the new location.
however you do realize that when a customer breaks a contract (via moving) it was not due to verizons doing. They may insist on your paying the early termination fee. Which is their right.
a waiver is not a right but a courtesy. For the record I never use a contract for goods and services outside of a retail service agreement on my autos, credit cards, or home.
I am still trying to get a form. Have tried to contact customer service twice so far and never received a form.
