ETF Waiver ISSUE

I'm having the same issue as multiple other people. The link I was sent will not allow me to upload my new lease so that I can have the ETF waived. I've tried two different browsers, deleting my cookies, talked to 7 different Verizon reps on the phone, and emailed twice. Please advise.
