Moved and early termination waiver

I have tried to use the site for the waiver. I was living in Rhode Island at the time and was told that the early termination fee would be waived. The reason was because was moving to a state that does not offer fios. I got the service in Dec. or Jan. and moved in March. Now it's in collections and I don't think it's fair. I talked to someone on the chat feature and she told me to go to the waiver site and that's what I did and it's not letting me submit. Please help!!!!!!
