$50 Rewards card for Verizon FIOS service

I've received an email saying I am running out of time to redeem my $50 Visa prepaid card after I purchased the Verizon Fios internet service. However, there is no button to redeem the rewards card.

It seems this is a widespread issue. Someone please help
