$400 bonus/free iPad/phone voicemail

i joined Verizon 8/14/2015, I called after receiving the fios flyer (7dc) with the 400$ bonus And the sales person said I would receive a free iPad. To date I have not received either. When will I get them?

also, my home number {edited for privacy}, when I call {edited for privacy}to setup voicemail it states I do not have a valid phone number so I can not get it setup and I don't know how I should proceed.
