How Do I Disconnect Equipment and Reconnect It After Move?

So I am about to move and have put in an order for a change of service on June 6.  The order was for me to disconnect the equipment from the old address and reconnect it to the new which is already wired for FIOS. How do I disconnect the old equipment and how do I reconnect it?

Based upon the information here, it seems that Verizon would install new equipment in the new home (I haven't moved while being a FiOS subscriber).

If the situation has changed, someone will provide updated information. 🙂
